Description: Seeking a dedicated and compassionate Nurse to join our team. As a Nurse, you will play a crucial role in providing community case management support to our clients and ensuring they receive the highest level of care. You will serve as a liaison between patients, their families, and healthcare providers, addressing concerns and promoting effective communication and collaboration.

Responsibilities:
1. Medical Advocacy: Serve as a strong advocate for clients and their families.
2. Care Coordination: Assuring clients needs are addressed.
3. Health Education: Provide clients and their families with accurate and understandable health information.
4. Client Support and Assistance: Offer emotional and practical support to clients and their families, helping them navigate complex healthcare systems.
5. Ethical Decision-making: Ensuring adherence to ethical principles and standards of care.
6. Documentation and Reporting: Maintain accurate and confidential records of client interactions.

Requirements: Experience: 3 years; Bachelor degree; 1. Education: Possess a valid nursing license and hold a degree in nursing; minimum LVN, prefer RN.
2. Experience: Prior experience as a case manager or in a similar role is preferred. Clinical experience in a healthcare setting is required.
3. Communication skills: Excellent verbal and written communication skills, with the ability to effectively communicate complex medical information to clients and their families.
4. Empathy and Compassion: Demonstrate a deep level of empathy, compassion, and understanding towards patients and their families.
5. Critical Thinking: Strong ability to assess situations, identify potential issues, and develop appropriate solutions.
6. Interpersonal Skills: Good interpersonal skills to build rapport and establish trust.
7. Organizational Skills: Strong organizational abilities to manage multiple priorities, prioritize tasks, and meet deadlines in a fast-paced environment.
